Valor PatientCaretm Program - Our Philosophy of Care is to Set and Exceed Standards in Patient Care

Valor HospiceCare is about exemplary care by our extraordinary associates and volunteers. Our hospice services are provided for compassionate, medically-directed hospice home care for patients' emotional, physical and spiritual desires. We also create meaningful and special moments for patients and families. Whether it be a visit by a volunteer for our pet therapy program, or music provided for memories, we are here to help.

Covered Comprehensive Services:

When our services are chosen, individualized care plans are developed, managed and utilized to treat each patient's and family's special needs. In fact, our patients are involved with the plans that we design with them. The patient's doctor, along with the Valor team, the patient and Medical Director, will determine the initial course of treatment and review the plan regularly in order to assure that the patient's changing needs are met.

In addition to regular home visits by a nurse, the following additional services for the hospice diagnosis are provided.

· 24-hours a Day On-call Care & Support
· Physician Directed Services
· Advanced Registered Nurses and Nurse Practitioners
· Expert Pain and Symptom Management
· Medications, Equipment & Supplies for Terminal Diagnosis
· Hospice Aides (C.N.A.)
· Medical Social Services
· Spiritual Care Providers
· Physical, Occupational, Speech and Respiratory Therapies
· Registered Dietitian
· Authorized Inpatient & Respite Care
· Continuous Care at Home During Periods of Crisis
· Trained Volunteers
· Bereavement Services

Communicating with Persons of Limited English Proficiency & Communicating Information to Persons with Sensory Impairments

We meet and exceed federal and state guidelines for interpretations and communication service programs. Currently, we have associates who fluently speak Spanish, French, German and American Sign Language.
