Valor HospiceCare & The Valor Institute for Palliative Medicine is all about exemplary care by extraordinary staff and volunteers. And based on our service evaluations and feedback, a level of care and patient attention usually not seen in health care.

Led by our medical team, our "Philosophy of Care" Charter is focused on our unique, comprehensive and integrated approach of changing and enhancing the way hospice and palliative comfort care is organized, planned and delivered in our markets. As an innovative, academic-based leader in evidence-based palliative care, we are improving end of life care, reducing costs to the healthcare system and expanding access to hospice and palliative care in the markets we serve.

Care at Home or Wherever Patient Resides

The Valor team provides care wherever the patient is best cared for and comfortable. Services are offered in private homes, nursing homes, independent living centers, assisted living and/or long-term care facilities and in the hospital for authorized medical emergencies.

Anyone suffering from life limiting, terminal disease can receive our services. Our focus is on care, not cure.

We provide professional services regardless of age, gender, sexual orientation, racial or ethnic background, or religion. In cases where no insurance is available, our services can be provided regardless of the patients ability to pay. As a locally owned and managed company, our team includes friends and neighbors from the community who are able to serve patient and family needs quickly and efficiently--with the highest quality care.

Our Philosophy of Care - An Academic, Outcome-Based Approach to Practicing Medicine with an "Integrative Attitude"

Valor HospiceCare embraces the concept of practicing medicine with an integrative attitude respecting the contributions of body, mind, spirit, environment and lifestyle to the health of the individual. We are open to integrating practices from other traditions for patient comfort in our coherent integrative Valor PatientCaretm Program based on outcome-oriented medical knowledge. The Valor Institute for Palliative Medicine       

The Valor Institute for Palliative Medicine is commited to our investment in new employees, current staff and community health care professionals. The Valor Institute comprises:

1) Academics, Knowledge & Research Program
     · For new employees, we provide over 80 hours of comprehensive, by-discipline training. For experienced staff, additional in-service training is provided in our academic-based program.
     · With Provost and Director oversight, The Valor Institute for Palliative Medicine will educate health care professionals on evidence-based practices of palliative care. Further, innovative research is conducted for innovative approaches to palliative care.

2) Valor PalliativeCare - Palliative Medicine Physician Services Program
     · Physician home or facility visits for hospice eligibility evaluations, as well as palliative care visits for hospice patients.

3) Continuous Quality Improvement (CQI)/Quality Assessment Performance Improvement (QAPI) Program
     · Continuous Quality Improvement programs and all safety, HIPAA and regulatory compliance initiatives.
