Clinical Careers - Valor HospiceCare

IMMEDIATE OPENING! On-Call Nurse (RN) - Weekend - Sierra Vista Office
Valor HospiceCare is seeking an On-Call RN. This position would be to carry phone and respond to patient calls as scheduled. The On Call RN is responsible as a member of the interdisciplinary team to the Program Director for the provision of skilled nursing assessment and hospice care in order to maximize the comfort and health of patients and families consistent with Valor HospiceCare policies and procedures. Responsible for providing skilled nursing, intervention, supportive care and admissions in patients' homes for weekend, holiday, evening and night on-call duty in accordance with the Hospice Interdisciplinary plan of care for each patient. Occasional travel to regional locations may be required.

IMMEDIATE OPENING! Nurse Case Manager (RN) - Full Time - Tucson Metro Office
Responsible Registered Nurse as a member of the interdisciplinary group for the provision of skilled nursing assessment and hospice care in order to maximize the comfort and health of patients and families. Currently registered in the state of Arizona. Minimum two years recent varied work experience as a professional nurse in an acute, oncology, home health or hospice setting. Provide community outreach, education and referrals for census growth. Must have good computer skills as well. Spanish speaking a plus.

Certified Nurse Assistants (CNA)
Due to growth we are in need of a Certified Nurse Assistant to work with our hospice patients. Under the direction of a Registered Nurse, provides personal care services within the guidelines of the interdisciplinary group plan to patients enrolled in Valor Patient Care program. Positions may be Full Time, Part Time or PRN.
A Certified Hospice Aide must be certified by the State Board of Nursing. The Aide must be current in health requirements. In addition, the Aide must meet federal home Health Aide training and competency requirements (Section 4021(b) of Public Law 100-203). This competency shall be demonstrated by passing a written competency and skills examination by a qualified Registered Nurse.

PatientCare Associates
The PatientCare Associate is responsible for assisting with admissions, marketing and community outreach as well as for clinical support services including medical records and supplies. Must be computer literate. Conduct intake of new referrals with marketing and clinical admissions staff. Coordinate new admissions with Physician, Admissions Nurses and Program Directors. Verify and update insurance coverage with Medicare, AHCCCS, managed Care and other beneficiary providers. Ensures PrivatePay Program is coordinated with clinical and social services staff. Tirage patient calls and notify appropriate personnel. Track and update daily census logs and prepare reports as needed. Particpate in CQI Committee. Order, track and cancel durable medical equipment for patient care. Track placement and completion of DME as ordered. Assist and support marketing team as needed, creating weekly and monthly reports of referrals, admissions, etc. Act as a patient advocate and support marketing programs in the community.

Medical Social Workers (MSW)
The Medical Social Worker is responsible for the implementation of standards of care for medical social work services. A degree in Social Work from an accredited educational institution is necessary, and a LCSW/LMSW is preferred. A demonstrated ability in casework, counseling and group work, including one year of clinical experience in hospice or health care setting, is required.
Maintains the standards of Social Work excellence in accordance with the principles set forth by the Arizona State Health Department, and agency policies and procedures. Performs comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of goal-directed family care plans. Observes, evaluates and brings to team conferences information regarding psychosocial, spiritual, physical and financial conditions affecting the patient and family. In addition to participating in marketing referral and admissions programs, assists with community outreach and census goals.

Bereavement Care Coordinators
The Bereavement Care Coordinator is responsible as a member of the interdisciplinary team to the Program Director for the provision of bereavement support to patient’s/family’s desire and belief system and consistent with Valor HospiceCare’s policies and procedures. Responsible for implementing and coordinating all activities of the bereavement services program, and the delivery of bereavement services to patients and families.   A bachelor’s degree or greater in counseling, social work or related field is required with a minimum of three (3) years experience in a clinical work, preferably either in a healthcare or hospice setting and demonstrated ability in casework, counseling and group work.

Nurse Practitioner duly licensed under the provisions of the Arizona Business and Professions Code. Masters of Science in Nursing (Phd). Minimum two years of geriatrics Nurse Practitioner experience required. Must be well organized, computer literate and well respected in the community.

Responsible to the Executive Director & Administrator for assigned functions related to business development, Interdisciplinary Group participation, admissions of patients, provision of services information, and liaison with doctors, inpatient and nursing home/assisted living institutions and groups identified as sources of referrals. Through business development, responsible for the initial patient identification, planning and coordination of admission of total care of patients to Valor’s hospice program of care. For community outreach supports Marketing Program, and through business development meets patient admissions goals. Computer skills required. Skills and Experience include: Minimum four years of varied work experience in a hospice, home care of health care setting. Demonstrated ability to relate effectively with patients, physicians, community agencies and institutions. Demonstrated ability to communicate effectively with members of the hospice interdisciplinary group (IDG). Demonstrated experience in education and training. Demonstrated excellence in communication skills; written and verbal. Bilingual English/Spanish a plus.

Admissions Nurse (RN)/Community Liaisons
Responsible as a Registered Nurse member of the interdisciplinary group to the Executive Director & Administrator for assigned functions related to providing information about services to potential Valor HospiceCare patients and admitting patients for service.  Conducts admissions evaluations and assessments and assists with planning of Admission Department projects and strategy. Following admission, ensures smooth transition of new patients to primary nurse. Assist with continuity of care and discharge planning in hospitals including working with the patient care, social services and marketing departments. Must have good computer skills. Current license as a registered nurse in the state of Arizona. Minimum two years recent varied work experience as a professional nurse in an acute, oncology, home health or hospice setting. Demonstrated ability to assess and respond to the needs of patients and families in varied settings. Ability to cope with family emotional stress and tolerance of individual lifestyles. Valor Health is seeking a full time Controller. The Controller is responsible to the Chairman & CEO for the planning, coordination and management of the financial affairs and related operational activities of the organization in accordance with Valor HospiceCare and Valor Health Services policies and procedures. Directs the organization’s financial planning and accounting practices as well as its relationship with financial institutions, contract services, and managed care agreements by performing the following duties personally or through associates.
Qualifications include:
- Bachelor’s degree in accounting, business, hospital administration or related field preferred. MS/MBA/CPA a plus.
- At least five years of health care related accounting work experience, with at least three years of supervisory experience.
- Ability to supervise and train employees, including organizing, prioritizing and scheduling work assignments.
- Demonstrated ability in fiscal and business management in a health care setting.
- Extensive experience in budget preparation, capital and banking management, cash management, accounting principles and procedures, and systems development and administration in accordance with GAAP.
- Experience in analyzing accounting problems and development of effective solutions.
- Experience in Medicare Part A and Part B, Medicaid (AHCCCS) and managed care/commercial insurance billing and reimbursement revenue cycle management.
- Extensive knowledge of Sage Accounting Systems.
- Knowledge of the Microsoft Suite of Products a must.
- As a member of the Corporate Administration Services team, serves various hospice license locations with consultative approach to fiscal responsibility, as well as provides financials services to Valor Health companies for palliative consultation services and primary home care.
- Demonstrated experience in being results and task oriented, a leader, team manager and an extremely fast learner with ability as a strategist to analyze and resolve issues.
