The Medical Social Worker is responsible for the implementation of standards of care for medical social work services. A degree in Social Work from an accredited educational institution is necessary, and a LCSW/LMSW is preferred. A demonstrated ability in casework, counseling and group work, including one year of clinical experience in hospice or health care setting, is required.
Maintains the standards of Social Work excellence in accordance with the principles set forth by the Arizona State Health Department, and agency policies and procedures. Performs comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of goal-directed family care plans. Nurse Case Manager (RN)

Certified Nurse Assistants (CNA)
Due to growth we are in need of a Certified Nurse Assistant to work with our hospice patients. Under the direction of a Registered Nurse, provides personal care services within the guidelines of the interdisciplinary group plan to patients enrolled in Valor Patient Care program. Positions may be Full Time, Part Time or PRN.
A Certified Hospice Aide must be certified by the State Board of Nursing. The Aide must be current in health requirements. In addition, the Aide must meet federal home Health Aide training and competency requirements (Section 4021(b) of Public Law 100-203). This competency shall be demonstrated by passing a written competency and skills examination by a qualified Registered Nurse.

PatientCare Associates
The PatientCare Associate is responsible for assisting with admissions, marketing and community outreach as well as for clinical support services including medical records and supplies. Must be computer literate. Conduct intake of new referrals with marketing and clinical admissions staff. Coordinate new admissions with Physician, Admissions Nurses and Program Directors. Verify and update insurance coverage with Medicare, AHCCCS, managed Care and other beneficiary providers. Ensures PrivatePay Program is coordinated with clinical and social services staff. Tirage patient calls and notify appropriate personnel. Track and update daily census logs and prepare reports as needed. Particpate in CQI Committee. Order, track and cancel durable medical equipment for patient care. Track placement and completion of DME as ordered. Assist and support marketing team as needed, creating weekly and monthly reports of referrals, admissions, etc. Act as a patient advocate and support marketing programs in the community.

Medical Social Workers (MSW)

Bereavement Care Coordinators
The Bereavement Care Coordinator is responsible as a member of the interdisciplinary team to the Program Director for the provision of bereavement support to patient’s/family’s desire and belief system and consistent with Valor HospiceCare’s policies and procedures. Responsible for implementing and coordinating all activities of the bereavement services program, and the delivery of bereavement services to patients and families.   A bachelor’s degree or greater in counseling, social work or related field is required with a minimum of three (3) years experience in a clinical work, preferably either in a healthcare or hospice setting and demonstrated ability in casework, counseling and group work.

Volunteer Coordinators
Responsible to the Program Director for all aspects of the Volunteer Program consistent with Valor HospiceCare’s policies and procedures. Plan and supervise delivery of all volunteer services for Tucson and Green Valley offices and service areas. Assign volunteers to service on the basis of program needs and the volunteers’ interests and skills. Assess and monitor a record-keeping system which includes services delivered and actual time involved. Recruit, interview and select volunteers. Design and supervise the orientation and training of volunteers. Monitor and evaluate volunteers’ performance. Assure volunteers’ compliance with agency policies and procedures. Plan and conduct volunteer support meetings. Prepare service reports as required by the program director. Provide liaison between community and staff, in addition to working with Marketing, Sales and Admissions for referrals to Valor HospiceCare’s program. Facilitate community awareness and support of Valor HospiceCare volunteer program. Maintain relationship with other program coordinators. Bachelor’s degree or equivalent experience. Demonstrated experience in related field and in volunteer activity. Demonstrated skill in administrative functions including ability to develop policies and procedures. Two or more years of supervisory experience. Demonstrated skill with written and oral communications. Demonstrated ability to execute a supportive and sensitive approach to volunteer and patient/family needs.

Marketing, Sales & Admissions Careers - Valor HospiceCare

IMMEDIATE OPENING! Regional Sales & Admissions Director - Full Time - Tucson Metro Office
The Regional Sales & Admissions Director is responsible to the Executive Director & Administrator for the overall development of the hospice sales program top line revenue growth and admission team and processes, through direct community contacts for the purpose of educating healthcare providers as well as the general public about the hospice program. The Sales and Admissions Program is part of the Marketing Department. Additionally, the Regional Sales & Admissions Director serves to coordinate the sales and admission nurse team's participation and execution of the program's strategic plan and serves as a resource for all staff.  
RESPONSIBILITIES
-Responsible for coordination of the Marketing Department’s Sales & Admissions Program, and the orchestration and implementation of community outreach, referral development and admissions initiatives for Southern Arizona  (Locations currently include Tucson Metro, Green Valley (Multiple Location) and Sierra Vista).
STRATEGIC PLANNING
-Supervisor responsible for all Sales staff, Community Liaisons and Admissions Nurses in the Southern Arizona Region.
-Participates with Executive Director & Administrator, in collaboration with the Program Directors, in the development of new value-added services and markets. Strategize and develop a competitive, hospice oriented Sales/Admissions Plan and implementation process within the Company.
-Develop initiatives to enhance the Valor HospiceCare brand in the marketplace.
Conducts and participates in weekly development meetings; completes and submits, as required, all activity reports and documentation.
-Maintain Sales & Admissions Gantt Chart up-to-date and applicable team members informed of any/all changes.
-Leads and Directs Marketing, Sales & Admissions Council strategic planning sessions.
-Support management team and corporate administration services in crafting long-term strategies for Valor HospiceCare’s key growth areas.
SALES
-Coordination of the Sales management program, including developing strategic sales strategies, metrics, research, communications, the development of sales tools and the monitoring of educational needs for the sales team to enhance their professional development.
-Maintains a strategic and organized approach to territory management; directs preparation and conducts calls and presentations to potential referral sources. Participates in sales forecasting, strategic planning and the analysis for all assigned program territories in conjunction with the hospice agency business plan.
-Identify potential business opportunities in new markets.
-Drive increased penetration with existing business partners.
-Develop metrics, processes and analytics to create a data and customer research driven decision-making culture.
-Satisfactorily meets and/or exceeds quarterly Management by Objective (MBO) referral/admission goals.
-Provide measurable top-line revenue results for the sustained growth of the Company’s average daily census.
-In accordance with the Executive Director & Administrator develop annual projected, Admission and Sales budgets for Valor Hospice Care’s Marketing Department Program.
-Maintains current data on market area, competitors, and sales strategies.
-Prepares a goal directed development plan for the purpose of educating health care providers and the general public about the hospice program.
-Collaborates in insurance payer credentialing programs and establishes reimbursement rates in accordance to company cost of sales and exceptions reporting metrics.
-Create and perform education presentations on quality and benefits of choosing Valor HospiceCare; provides accurate information regarding hospice services in response to inquiries by healthcare providers and general public.
-Ensures a steady inventory of necessary marketing/sales supplies for each location, including constantly researching cost effective means of acquiring supplies.
-Send Physician letters when appropriate, in a timely, professional manner.
-With Program Directors, and as part of Valor HospiceCare’s Continuing Education and Leadership (CEL) Program, provide quarterly Sales trainings to all clinical staff.
-In collaboration with Program Directors, directs PatientCare Associates (PCAs) with Inside Sales Program.
-Collaborates with Marketing Communications (Marcom) for marketing tools and utilizes guerrilla marketing techniques for adding valuable, cost effective Marketing Programs, identifying and implementing low cost and/or no cost marketing strategies and opportunities.
-In concert with the Executive Director & Administrator, Controller & Operations Director and Contracting, assists in development of partnership agreements/contracts with healthcare systems and community-based healthcare providers.
-Assists with valuable community outreach sales tools, including the production of press release materials and media campaign ideas.
-In coordination with the Continuous Quality Improvement (CQI) Program, implement measures to improve overall operational objectives, in accordance with Medicare conditions of participation.
-Other assignments as may be appropriate.
ADMISSIONS
-Oversees the Valor Admissions Protocols and Processes, in collaboration with Medical Directors and Program Directors, and with support of Quality, Compliance & Education.
-In collaboration with the Program Directors, trains all admission staff and clinical staff on hospice benefit and Valor admission protocols to increase admission rates.
-Process referrals by sharing and obtaining information to assure pre-qualifying of patients efficiently.
-Establish positive relationships with patients and families.
-Obtain complete and accurate information to facilitate the admissions process and to maximize the likelihood of reimbursement for our clients.
-Communicate effectively with referral sources and team members to support the business partnerships developed by the sales team.
-Establish positive relationships with insurers and managed care entities: third party payers, case managers, etc.
-Analyze referral/admissions data and prepare assigned reports.
-In accordance with policies and procedures, ensure compliance with state and federal regulations, LCDs/LMRPs, CAP measurements and Valor Philosophy of Care Charter.
-Provide information regarding Valor HospiceCare’s unique Patient Care Program and services to patients, physicians and other sources of referrals.
-Participate in Valor HospiceCare’s community outreach/education efforts utilizing Valor HospiceCare’s clinical staff and volunteers as appropriate.
QUALIFICATIONS
-Bachelor’s degree and/or equivalent experience and knowledge of sales management. Registered Nurse (RN) optional.
-Minimum of five (5) years sales and leadership experience in the southern Arizona community.
-Confidence and demeanor to gain respect of internal and external healthcare system executives.
-Demonstrated ability to communicate within high levels of agency.
-Proven experience in referral development plans and admission policies.
-Demonstrated ability in program administration including extensive knowledge of Medicare Conditions of -Participation, state licensure regulations, health care policies and reimbursement practices.
-Prior experience with HMO/PPO/Advantage partnership agreements a plus.
-Very professional and creative thinker with the ability to develop and explain agency service offerings to staff and health care partners.
-Demonstrated results in designing, coordinating and implementing strategic sales plans, in accordance with overall business plan of agency.
