IMMEDIATE OPENING! Program Director & Administrator - Full Time - Tucson Metro Office
Responsible to the Executive Director & Chief Administrative Officer (CAO) for all aspects of the patient services program consistent with Valor HospiceCare policies and procedures. Responsible for the overall quality, compliance and education functions of the organization.  In collaboration with the Program Directors & Administrators, responsible for managing the day-to-day quality, compliance and education functions of the organization. Directs CQI Associates for orchestration and implementation of Valor’s CQI programs, participates as a member the Executive Leadership Team as requested, and participates in periodic leadership meetings as a member of Valor’s senior leadership group. This position is regional in scope, and may involve some travel around the Valor service area as needed for QCE functions.

QUALITY, COMPLIANCE & EDUCATION RESPONSIBILITIES

  • Oversees Quality, Compliance & Education Department with Program Directors & Administrators support of all locations.
  • Oversee continuous quality improvement (CQI) program with the Program Directors as team lead, to implement measures to improve overall operational objectives, in accordance with Medicare conditions of participation.
  • Oversee all specified Medicare scheduled meetings for all Valor HospiceCare licenses for Continuous Quality Improvement (CQI) and The Valor Institute for Palliative Medicine.
  • Oversees all Valor quality initiatives, and acts as lead Compliance Officer for Program integrity.  Has ultimate oversight responsibility for all CQI meetings for each Valor HospiceCare program.
  • Oversees the Safety Officer in each Valor HospiceCare program; has overall accountability for Safety Compliance programs throughout the Valor organization.

Due to growth we are in need of a Certified Nurse Assistant to work with our hospice patients. Under the direction of a Registered Nurse, provides personal care services within the guidelines of the interdisciplinary group plan to patients enrolled in Valor Patient Care program. Positions may be Full Time, Part Time or PRN.
A Certified Hospice Aide must be certified by the State Board of Nursing. The Aide must be current in health requirements. The Medical Social Worker is responsible for the implementation of standards of care for medical social work services. A degree in Social Work from an accredited educational institution is necessary, and a LCSW/LMSW is preferred. Valor HospiceCare is in need of a Spiritual Care Provider available on a part time basis. The Spiritual Care Provider is responsible as a member of the interdisciplinary team to the Clinical Coordinator for the provision of spiritual support to patient’s/family’s desire and belief system and consistent with Valor HospiceCare’s policies and procedures. Responsible for implementing and coordinating all activities of the spiritual services program, and the delivery of spiritual services to patients and families.  Graduate of accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry and/or counseling. Licensed Practical Nurses (LPN)
Due to census growth Valor HospiceCare is looking for a LPN based on a PRN schedule. The LPN is responsible as a member of the interdisciplinary group to the Clinical Coordinator, assigned Nurse Case Manager (RN) and the hospice medical director for the provision of evaluation and care in order to maximize the comfort and health of patients and families consistent with Valor HospiceCare policies and procedures. Responsible for identifying patient/family needs and for providing supportive care and appropriate services, in accordance with the attending physician’s orders and the interdisciplinary group plan of care. Qualifications include possession of current license to practice as a licensed professional nurse in the state of AZ, a minimum two years of varied work experience as a professional nurse and preferably one of the two years in an acute or home health setting, demonstrated ability to assess and respond to the needs of patients and families in varied settings, and demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

PatientCare Associates
The PatientCare Associate is responsible for assisting with admissions, marketing and community outreach as well as for clinical support services including medical records and supplies. Must be computer literate. Conduct intake of new referrals with marketing and clinical admissions staff. Coordinate new admissions with Physician, Admissions Nurses and Program Directors. Verify and update insurance coverage with Medicare, AHCCCS, managed Care and other beneficiary providers. Ensures PrivatePay Program is coordinated with clinical and social services staff. Tirage patient calls and notify appropriate personnel. Track and update daily census logs and prepare reports as needed. Particpate in CQI Committee. Order, track and cancel durable medical equipment for patient care. Track placement and completion of DME as ordered. Assist and support marketing team as needed, creating weekly and monthly reports of referrals, admissions, etc. Act as a patient advocate and support marketing programs in the community.

Bereavement Care Coordinators
The Bereavement Care Coordinator is responsible as a member of the interdisciplinary team to the Program Director for the provision of bereavement support to patient’s/family’s desire and belief system and consistent with Valor HospiceCare’s policies and procedures. Responsible for implementing and coordinating all activities of the bereavement services program, and the delivery of bereavement services to patients and families.   A bachelor’s degree or greater in counseling, social work or related field is required with a minimum of three (3) years experience in a clinical work, preferably either in a healthcare or hospice setting and demonstrated ability in casework, counseling and group work.

Volunteer Coordinators
Responsible to the Program Director for all aspects of the Volunteer Program consistent with Valor HospiceCare’s policies and procedures. Plan and supervise delivery of all volunteer services for Tucson and Green Valley offices and service areas. Assign volunteers to service on the basis of program needs and the volunteers’ interests and skills. Assess and monitor a record-keeping system which includes services delivered and actual time involved. Recruit, interview and select volunteers. Design and supervise the orientation and training of volunteers. Monitor and evaluate volunteers’ performance. Assure volunteers’ compliance with agency policies and procedures. Plan and conduct volunteer support meetings. Prepare service reports as required by the program director. Provide liaison between community and staff, in addition to working with Marketing, Sales and Admissions for referrals to Valor HospiceCare’s program. Regional Sales & Admissions Director
The Regional Sales & Admissions Director is responsible to the Executive Director & Administrator for the overall development of the hospice sales program top line revenue growth and admission team and processes, through direct community contacts for the purpose of educating healthcare providers as well as the general public about the hospice program. The Sales and Admissions Program is part of the Marketing Department. Additionally, the Regional Sales & Admissions Director serves to coordinate the sales and admission nurse team's participation and execution of the program's strategic plan and serves as a resource for all staff.  
RESPONSIBILITIES
-Responsible for coordination of the Marketing Department’s Sales & Admissions Program, and the orchestration and implementation of community outreach, referral development and admissions initiatives for Southern Arizona  (Locations currently include Tucson Metro, Green Valley (Multiple Location) and Sierra Vista).

Admissions Nurse (RN)/Community Liaisons
Responsible as a Registered Nurse member of the interdisciplinary group to the Executive Director & Administrator for assigned functions related to providing information about services to potential Valor HospiceCare patients and admitting patients for service.  Conducts admissions evaluations and assessments and assists with planning of Admission Department projects and strategy. Following admission, ensures smooth transition of new patients to primary nurse. Assist with continuity of care and discharge planning in hospitals including working with the patient care, social services and marketing departments. Must have good computer skills. Current license as a registered nurse in the state of Arizona. Minimum two years recent varied work experience as a professional nurse in an acute, oncology, home health or hospice setting. Responsible to the Chairman & CEO for the planning, coordination and management of financial affairs and related operational activities of the organization in accordance with Valor HospiceCare policies and procedures. Directs the organization's financial planning and accounting practices as well as its relationship with financial institutions, contract services, and managed care agreements by performing the following duties personally or through associates. Directs the corporate administration services planning and shared services, in collaboration with Executive Director & Chief Administrative Officer and Program Director & Administrators for all locations, licenses and companies.
QUALIFICATIONS
-Bachelor’s degree in accounting, business, hospital administration or related field required. CPA a plus.
-At least five years of health care related accounting work experience, with at least three years of supervisory experience.
-Ability to supervise and train employees, including organizing, prioritizing and scheduling work assignments.
-Demonstrated ability in fiscal and business management in a health care setting.
-Extensive experience in budget preparation, capital and banking management, cash management accounting principles and procedures and systems development and administration in accordance with GAAP.

RESPONSIBILITIES
-Is a consultant to the Chairman & CEO and Governing Body and is able to effectively meet or exceed all deadlines and Business Plan goals.
-Oversees Corporate Administration Services operations for all day-to-day duties currently including the finance, human resources, information services, strategic projects departments and collaborates with quality, compliance & education department.
-As a member of the Corporate Administration Services team, serves Executive Directors & Chief Administrative Officer (CAO) and Program Directors & Administrators for Clinical & Marketing, Sales & Admissions Departments at various hospice license locations, with consultative approach to fiscal and operations responsibility, as well as provides financial and operations services to Valor Health companies for medical group palliative consultation services and primary home care.
-Responsible for the development and ongoing administration of the accounting system, business records, preparation of budgets with interdepartmental teams and financial reports.
-Responsible for supervision of payroll, billing, financial recording, fixed assets, accounts receivable, accounts payable and cash management; utilizing a very hands-on approach.
-Proactively appraises the organization’s financial position and issues periodic financial and operating reports.
-Preparation and issuance of the organization’s monthly and annual Financials.
-Directs real estate and insurance activities for the organization.
-Analyzes, consolidates, and directs all cost accounting procedures together with other statistical and routine reports.
