Referrals & Insurance Benefits Referrals and Electing Hospice Care The three steps to receiving hospice care include: 1) Are
you or a loved one ready for hospice care? If your doctor believes that further treatment is unlikely to alter the disease process, hospice care can offer the alternative of comfort care in your home. From physicians to friends, family members or neighbors, anyone can make a referral for hospice care. Valor HospiceCare & The Institute for Palliative Medicine team members are available by phone 24-hours a day, seven days a week. When a referral is made, a Valor HospiceCare representative will schedule a visit with the patient and/or their loved ones, to assess the best type of care options available. The person has a life limiting prognosis with six months or less to
live Paying for Valor HospiceCare Services Hospice is an 100% covered benefit of Medicare for services related to the terminal illness or disease and pre-authorized by the Hospice team. Valor is an AHCCCS/ALTCS (Arizona's Medicaid Program for Long Term Care) provider and accomodates most private insurance companies. We also offer additional payment options, including services for managed care (HMO, PPO, etc.) and private pay. In cases where no insurance is available, our services can be provided regardless of the patients ability to pay.
